A good overview of results is important to be able to assess whether the measures implemented are effective.
When the follow-up is to end, the professional can quickly and easily create access to the final mapping which the user can fill in on their own.
CLEAR AND TIME-SAVING
In a similar way as admission mapping, the professional receives a visual overview with development from the first to the last measurement, and a completed closing note.
The note can be copied into most record systems, reducing the workload with record keeping.
The note can also be used as a basis for feedback to the GP, or for further referral.
USER RETAINS ACCESS
When the follow-up ends, the user will retain access to the content they get access to in Assistert Self-Help for as long as they wish.
Access is always renewed with 12 new months from the last login.
